ENCOMPASS Transplant Management
ENCOMPASS offers Transplant Management services to provide individualized care interventions for members at all stages of organ or tissue transplantation.
This unique service is provided by ENCOMPASS nurses with the expertise needed to handle care coordination, provider collaboration and benefit considerations necessary for the unique and costly medical situations that occur with transplants.
Outcomes are improved through referral to established transplant centers throughout the U.S. and Case Management care coordination with facilities, providers and health plan administration. Savings are seen through contracted transplantation network rates and care coordination oversight of services.
Customizing Solutions
Our Transplant Management service is one of the full range of ENCOMPASS Health Management services offered. We work with each client's health plan requirements to build a customized profile from our comprehensive Care Coordination and Medical Management solutions.
As with all ENCOMPASS services, Transplant Management case information is completely integrated in our proprietary software system, QualiTrac, which supports documentation and reporting for all programs. QualiTrac is a member-centric system making information available to ENCOMPASS medical professionals to address health status and medical needs across all Care Coordination and Medical Management programs.
